(1) As soon as practicable after an examination has been completed, the Chief Examiner must give a written report to the Special Investigations Monitor, setting out—
(a) the reasons for the examination and the place and time of the examination; and
(b) the name of the witness and of any other person who was present during the examination; and
(c) the relevance of the examination to the organised crime offence in relation to which the coercive powers order was made; and
(d) any other prescribed matters.
(2) A report under sub-section (1) must be accompanied by a copy of the video-recording made under section 45 and, if a transcript is prepared, a copy of the transcript.
