An authorisation under
section 152(1) must —
(a)
state that the authorisation is given under this Division; and
(b)
generally describe the serious public health risk to which it relates; and
(c) if
the serious public health risk has arisen, name or describe the place at which
the serious public health risk has arisen; and
(d)
specify the time at which the authorisation is given; and
(e)
specify the serious public health incident powers that may be exercised under
the authorisation; and
(f)
specify the period during which the authorisation continues in force.
